reimbursement rates. Standard office visit gets reimbursed by single payer system at say $50/visit. If you’re a doctor, you can make a volume practice by only charging the single payer reimbursement amount, or be more of a concierge practice by charging an annual fee, and/or an upcharge on the standard reimbursement. But in both cases I don’t see why a volume or concierge practice shouldnt both get the standard reimbursement rate for the visit from single payer. The concierge patient is paying into the single payer system (probably at a much higher rate too)
