Either private insurance or medicare/medicaid. We don’t know what % of the rest actually wants to be in the mainstream healthcare system, but let’s assume it’s 80% - or 12 of the 15% that lived in the coverage gap.

I reject the idea that - whatever the solution is - that it was not to first try to address that 12% instead of revamping the system for 100%.

There also has to be SOME element of making people own their decisions — and the only ways to do that are either increased cost or decreased access to care. We know what makes you unhealthy … smoking, excess sugar, saturated fats, drugs, alcohol, lack of sleep, etc. If you want to partake in those things, there’s no reason people who make good decisions should be forced to subsidize those decisions. That’s not to say that somebody who comes up unlucky and is born with a genetic heart condition or gets brain cancer or whatever condition shouldn’t be taken care of in the shared risk pool. But if you smoked for 40 years, yes it’s fucking unreasonable to ask society to pay for your lung cancer treatment. If you ate 5000 calories and 200g saturated fat every day for 40 years, you want a bypass surgery that requires hundreds of man hours by highly trained medical experts covered by tax payers? Fuck you.
