There is a lot of waste and inefficiency in our current system. I actually
envision more draconian reimbursement rates than medicare. Providers can accept it or charge a premium on top. Some providers will organize around throughput and efficiency, with longer wait times, more use of PAs/NPs, etc and accept basic reimbursement. Others will charge a premium for higher levels of service, shorter wait times. Employers could even offer premium insurance which sits on top of medicare for all and allows for better care. This would result in a two tier system, but we already have that.
I would fully expect to pay out of pocket for better care under such a system.
