There is a maxim in the recovery community that asserts that the addicited

individual needs to reach "rock bottom" before they reach out for help. Of course rock bottom looks very different to every individual...but for the person living in a tent because of addiction they have opted for that life to continue their substance use. They may truly want help, but the addiction is still winning the daily struggle. That is a particularly challenging group to help...but some do escape.
