Biggest reform we need is health care related spending... between Medicare
medicaid, ACA supplements, VA, etc. The ever growing cost of healthcare is what is ultimately going to do us in. Social security could use some tweaks but is in surprisingly good shape overall. And even with all the geopolitical instability, I still think we need to cut military spending. Obviously we need to continue to invest to maintain our technological superiority, but I'd bet there is a lot of spending still related to waging massive conventional warfare that is just wasteful.
All other govt departments get an immediate 10% cut (figure it out) with a larger reduction targeted over some period of time (5-10 years).
Maybe some tweaks to tax rates.... restore previous top marginal rates (39.6% top rate). We need more revenue, but I think trying to do too much, too quickly would be counter productive. Ultimately we are going to need to grow, not tax our way out of our massive deficits on the revenue side.
