Back in the 70s VA was trying to recruit MDs for a grunch of counties ...

that didn't have one (notably Goochland between C'ville and Richmond). There was a proposal that the county would pay for their medical training; give them a house, a car, and a base salary; and they could keep any fees that they managed to collect. In return, they would have to stay and work in the county for X amount of time per year of money. Never went anywhere, though.
