Demand for services exists without insurance also
if you have something seriously wrong with you you seek treatment. Part of the overhaul (regardless of whether it was successful or not) was to stop uninsured from relying on emergency room visits for their treatment. More coverage presumably increases demand for basic services that the uninsured would not have bothered with before, but I am not sure how closely it ties to demand for serious treatment. I haven't seen an analysis of the numbers.
